Diving and Course Forms

• Mango’s Dive Center – Divers Record and Information Sheet – (Diving with us?  Fill this out!)
• Boat Travel and Scuba Diving Release – (This is the release you will fill out to dive with us)
• Diver Medical – Participant Questionnaire w/Physician Approval  –  Medical Questionnaire required with all courses.
• DAN Health Declaration Form – Required from all divers
• Continuing Education Release Document  – (Reactivate/Scuba Review, Advanced Open Water, Specialties, Rescue Diver – this is the form you will need to complete)
• Enriched Air (Nitrox) Diver Training Release – (Release for the PADI EAN Course)
• Youth Responsibility Release  – (Are you under 15 years old?  We need this signed by you and your parents or guardians before you can take a class with us.  You must also watch the PADI Youth Diving Video.)
• “Try Diving” Information and Release – The information and release that we need for you to take part in one of our “Try Diving” pool sessions.
• Master Scuba Diver Application
  The application you submit to us for your Master Scuba Diver Rating.
